An advance directive conveys the health care wishes of an individual while they are still mentally capable of expressing them. Presuming the individual is, in fact, incapable, the wishes of the patient, when expressed, are paramount and generally binding when in opposition to the position of the medical practitioners (such as in end-of-life cases and withdrawing life support). But they should not be applied mechanically or literally without regard to relevant changes in circumstances.
